I am in a process of implementing a User Account Requisition form and i need some input.

Last Name
Residential Address
Home Number
Mobile Number
Department
Company
Manager
Do you require remote access?
Do you require an email address?

Rules and regulations.

I need to tell my users that:

1.All floppy drives will be disabled
2.Users will be allocated 200MB space on network
3.e-mail traffic will be monitored,certain mail attachment will not be allowed

What else should i include?
